In the PT aggregate does whats checked in regards to drift matter? also clock source?
Pro Tools aggregate? What exactly are you trying to aggregate? There is likely the problem. Try using the interface directly as the playback engine. And try using built-in output directly as well. Are they both stable?

Uncheck ignore errors in the playback engine dialog while doing all this so you see any AAE errors.

If that is not it...

Is the Mac fully optimized every last thing done?

What exact standard troubleshooting steps have you tried, esp. Trashing prefs? Removing all third party plugins etc. under "help us help you" up top of every DUC page.

"External disk" tells us nothing useful. What exact make/model/spec disk? Connected exactly how?

OS X version? El Crapitan?... Well bad mistake.
