Default .midnam step-by-step install instructions (OSX)

Would some one be so kind as to detail out the exact (and correct) steps to installing a .midnam file in OSX. I have successfully done it sometimes and then not other times doing the exact same steps; not quite sure what additional steps I'm missing.

Another question. Why are there only a few instruments that show up in the MIDI setup in OSX. Irrespective to any DAW software installed, there is the MIDI setup utility. How do we get our instruments in there. If I install PT, I see some. When I install Cubase SX, more show up and when I installed DP4, more showed up. Does the MIDI setup in OSX use the same .midnam file as these programs or is it separate? See, I knew all this in OS9, but have not taken the time to dig deep into OSX yet. Can someone clue me in?
