I'm having these same issues in PT 2019.5 / 19.5 Eucon / Mojave, and was having all of these issues in the last version of PT under High Sierra.
Also... I'm having an issue where I insert a send on a channel and when I try to adjust the send (in flip mode, not sure if it'll do the same when not flipped), the fader will not adjust the send or vice versa. very annoying and this controller is starting to be unusable. Channel shows that there is a send on the channel but acts like there isn't.
