Are you very sure you have the cards connected correctly? Ribbon cables must go from B to A and not from A to B as you would guess. Double-check that and reverse the ribbon if that is the problem.

(if the ribbon cables are backwards, then the first interface has to be connected to the last card. that too would work, but is unsupported configuration)
