Yes, as Brad mentioned, detailed trouble-shooting is best. Looking back, my situation was in fact a bad flex cable -and another time an inadequate power supply (under-powered) on a "home brewed" PC...

If I'm not mistaken (which you might have already tried), you have to also setup your MIDI device in OSX by going to "Applications" then "Utilities" then "Audio MIDI setup". Not real...
