It seems to happen when the Selector Tool bar is over a note and I grab a different note with the Grabber Tool when it's set to Separation Grabber.

Yes I've been having this incredibly frustrating issue for a couple of months. I tried to get it solved with PT support and after un-installing, re-installing etc for weeks have now been bumped up...
