Hmm, lots to think about(still)
1-A single 192 can do 16 inputs at 96K. Adding an input card to a "standard" 192 covers that need(unless you have something that can connect to the AES inputs)....

Depends....is this a recent addition that has never worked before? If yes, have you set it up under Setup>Midi>Midi Studio(menu) as a new "instrument"?

1-(my opinion) An HD/Native card is probably a good option(add a pair of used 192 IO boxes for 16 analog inputs @96K)

Its down to the source verses the track. Sounds like you are trying to record a MONO source(sound) onto a STEREO track and that's not how it works. Record a mono source to a mono track. Some examples...
