same here - title fill fails and simultaneously the keyboard shortcut highlight will fail to light, and soon after keyboard shortcuts fail to work. Only answer is quit/reopen. sometimes i restart...

have you tried turning off Edit Insertion Follows Scrub/Shuttle?
toggle cmd N with no QT chase to eliminate that variable.
