1. Slots (as you're looking at the 192 from the back) 1st input card in top left slot, 2nd input card in bottom right slot.

Hi Darryl, yes I did the default IO in Pro Tools. The problem is still there. I can't do screen shots from my Mac - internet signal isn't good enough, sorry.
