I have this same problem with my two 003R and Mac OSX 10.8.4.

I also have the problem of overloading the CPU (20-30% more) when I use the 003R or 003R+.

Avid has never solved both...

I have not yet found a solution. Technical Service Avid wrote me that this error is not known.

Please Avid, try quick fix this problem. It can not work properly with our PT11 and 003R.
