First thing I see is that you don't have a separate qualified drive for your audio sessions and the system drive you're currently using is too slow (5400rpm).

May be I've missed it, but what exactly are the problems you are having that are making PT unusable? I mean, you say you're are having these fragmentation issues, but what specifically is the result?

In particular, Internet Explorer 8 elements did not interact with some audio plug ins correctly... literally just having IE 8 installed, whether you were actively using it or not. Not just Avid...

No issues with Chrome, no issues with Firefox either. Also understand, the problems with Internet Explorer involved IE 8 with versions prior to Pro Tools 8... and it wasn't just PT. Waves plugins for...

I still suspect that IE 8 is a prime culpret as that 0xc0000005 error was typical.
What exactly happened when you attempted to upgrade to IE9?

What ever your previous version was is now installed. You cannot remove all...

I agree with albee in suspecting your drives. The speculation online is that many My Books come with 5400 rpm drives.... some of them are "green" drives. Neither would be good for PT sessions.
