just chiming in here, with the new driver update my headphone out problem is gone, but the dsp and the tuner are also non functional.

running mountain lion 10.8.1 with the mbox pro config...

I have this same exact problem. I use an m-audio axiom pro 61 and an mbox 3 pro. Every time I relaunch audio midi settings, there is a duplicate greyed out mbox pro icon. This makes it impossible...

I just purchased a brand new Mbox Pro (3rd gen) in Mid October. I experience all the issues mentioned previously:

1) headphone cutouts (a and b)
2) when the headphones do finally work the levels...

Well after two days of googling I found this. It's from 2006 and relates to some guy trying to drag midi into logic. So another Mac person has had something similar happen. Albeit years ago... Sigh...

I see a little rectangle when dragging and then nothing happens in pt. Pt doesn't even acknowledge where I'm moving the mouse by showing the start position of the groove/midi if that makes any sense....

I'm having a really weird issue. Running pt 9.02 in osx 10.6.7, I seem to have lost the ability to drag midi files from finder onto a midi/instrument track. This goes for any midi, including grooves...
